Please follow the information below for certificate replacement for both Defensive Driving and Pre- License programs.

MV- 278 Certificates are good up to one year of class completion.  For replacement of certificate, please send us a self stamped envelope with the following information including a $5.00  replacement fee. ( Checks payable to Mid-Hudson Safety).
Please include the following:
            Name and Address
            Location and Date of class taken
            Permit ID # ( including eye color)
            Date of Birth
            Phone Number ( for contact)

If your certificate is over a year, it has expired.   You will need to repeat the Pre-License course.

Defensive Driving certificates take 6 weeks for processing.  If you need confirmation of completed course immediately,  please contact MHS  through email - mhs@midhudsonsafety.com  with your name, motorist ID number, location and date of class, and a contact number to call. Once your attendance is verified- MHS will issue a letter for class completion.
This letter may not be used in replacement of certificate.
